Frequent Inspection — Operator

Who: Operator or competent person  ·  1910.179(j)(2)

Visual + functional pre-shift walkdown. Check operating mechanisms for maladjustment, air/hydraulic system leaks, hooks for deformation and cracks, hoist chains for wear/distortion, running ropes for visible damage, brakes, limit switches, controls, warning labels. Record only on written report if defect found and equipment tagged out.

Tier 2 Monthly + 1-12 Months

Periodic Inspection — Qualified Inspector

Who: Qualified inspector  ·  1910.179(j)(3)

Full structural and mechanical exam: deformed/cracked/corroded members, loose bolts and rivets, cracked or worn sheaves and drums, worn or damaged brake parts, chain drive sprocket wear, electrical component and controller condition. Signed certification record — inspector identity, date, equipment serial number, findings. Frequency 1-12 months based on service severity; steel plant Class D-F cranes at the demanding end.

Tier 3 Before First Use

Initial Inspection + Load Test

Who: Qualified inspector  ·  1910.179(k)

Before first use, after major modification or repair, and after significant structural change. Load test typically at 100-125% of rated capacity per manufacturer specification or applicable ASME B30 standard. Full documentation retained for the operational life of the crane.

Wire Rope Removal Criteria

The Money Numbers — When Wire Rope Comes Off, Not Later

Wire rope drives the majority of overhead crane findings. ASME B30.2 specifies discard criteria rather than fixed replacement intervals — the wire rope stays in service until it hits one of these thresholds, then comes off. Below is the working criteria stack.

Broken Wires — One Lay

6 wires

Six or more randomly distributed broken wires in one rope lay = mandatory removal. Count during Tier 2 periodic inspection.

Broken Wires — One Strand

3 wires

Three or more broken wires concentrated in one strand of one lay = mandatory removal. Concentrated failure signals localised fatigue.

Diameter Reduction

1/3 nominal

Rope diameter reduced beyond one-third of nominal = remove. Wear from sheave contact or corrosion. Measure with calipers.

Physical Damage

Any

Kinking, birdcaging, crushing, heat damage from electrical arc contact = mandatory removal regardless of wire count. Structural integrity compromised.

The Hook Removal Criteria

Hook Throat > 15% or Twist > 10° — Remove From Service Same Shift

Hook wear is the second-most-cited overhead crane finding after wire rope. ASME B30.10 removal criteria are specific: throat opening more than 15% above manufacturer's original dimension, twist more than 10° from the plane of the unbent hook, visible cracks anywhere on the body, or missing keeper plate (spring-loaded safety latch). Any single criterion = tag out of service. Measure throat with calipers on periodic inspection, not by eye.

Overhead Crane Inspection Questions

Do Tier 1 pre-shift inspections require a written record every day?

Not unless a defect is found. OSHA 1910.179(j)(2)(i) requires the operator to inspect the operating mechanisms daily but only requires a written report when a defect is discovered — the operator tags the crane out of service and generates the report. What's the difference between a competent and a qualified person?

A competent person can identify existing and predictable hazards and is authorised to take prompt corrective measures — typically the trained crane operator or a supervisor. A qualified person has a recognised degree, certificate, or professional standing and is authorised by written employer designation to perform the task. Tier 1 frequent inspections require a competent person; Tier 2 periodic inspections require a qualified person for the certification record.

How often must periodic inspections run?

1 to 12 months depending on service severity, per 1910.179(j)(3). Standard-service general-industry cranes (Class A-C) commonly run at 12-month intervals. Heavy/severe-service cranes (Class D-F — steel plant EOT, ladle, charging cranes, foundry service) must be at the demanding end — monthly periodic inspections plus daily frequent. How long must overhead crane inspection records be kept?

1910.179 does not specify a hard retention period, but industry best practice and ASME B30.2 guidance: monthly certification records for 3 months minimum, annual periodic inspection records for 12 months minimum, and load test / major repair / incident records for the operational life of the crane.
